The Learning Community for Person-Centered Practices

TLC-PCP is an international community that envisions a world where all people have positive control over the lives they have chosen for themselves. This site helps us foster a global learning community that shares knowledge for that purpose. It’s a place to find resources and trainers in person-centered thinking and person-centered practices.

Charting the LifeCourse

The Charting the LifeCourse framework was developed as a means of helping individuals and families of all abilities and ages create a vision for a meaningful life. Through this framework individuals and families are able to focus on a person’s current stage of life, including goals, aspirations and support needs. Charting the LifeCourse also focuses on early intervention and planning for the future as a way to accurately predict future support needs. LifeCourse offers numerous resources designed at helping individuals maximize their capacity to achieve self-determination, integration and inclusivity in all aspects of community life.

It's My Choice (PDF)

This planning tool provides a framework for helping people with IDD, plan for the life they want. This guide supports decision-making by focusing on what the person needs, plan for their future and determine what resources are available to support them. It includes activities and various checklists, designed to help a person consider all aspects of their life. This can be a useful tool for people with IDD, families, educators, service providers, case managers and other supporters.

National Wraparound Initiative

Wraparound is a planning process that seeks to help children and their families realize their hopes and dreams, while being a part of their chosen community. It is rooted in the principles of self-determination. This user guide is intended to serve as a road map for families. It provides strategies and offers various activities geared at the discovery process.

Inclusive Solutions

This organization offers a range of inclusive oriented services including casework, person centered planning, training, consultation and community building. Their team of psychologists and associates specialize in inclusion for people with disabilities as well as children and adults with challenging behaviors. Their website offers a full list of training opportunities as well as various resources

Copeland Center for Wellness and Recovery

The Copeland Center is the international leaders of Wellness Recovery Action Plans which are self-directed peer facilitated person-centered planning process that is evidence-based to support mental health recovery. These approaches can support anyone in wellness and recovery. The websites has resources, courses, and links to local leaders.
